All interested applicants to the PSU Graduate Programs are invited to submit the documents listed below in order to be considered for enrollment:

1. Duly accomplished application. Use prescribed form

2. Transcript of Records (TOR of Master's Degree for PhD or Bachelor's Degree for Master's applicants)

3. Certificate of Transfer Credential (Graduate / Transfer)

4. 1 x 1 recent facial photo, white background, decent attire, no hat/cap

5. PSA-issued Marriage Contract (for married female)

6. PSA-issued Birth Certificate

7. Two (2) referrals from former professor/s or current employer/supervisor. Use prescribed form

8. Research Proposal Concept Note in line with program applied for. Use prescribed form
